Not if he isn't established running (not going to the ground) with possession. Just like the Dez play.

Dez was outside the end zone prior to losing possession. If you're in possession as a runner and cross the goal line, it's a TD. NFL is inconsistent with their determinations based on field position.

That's a good idea. Kinda like tie goes to the runner. Give them the benifit of the doubt and if it's wrong over turn it.

I think it might be because they are paid to try to get the call right the first time, not what's convenient for the coaches.
